Hospice Coordinator (Part-Time)

Gallagher Hospice is looking for a caring, organized Hospice Coordinator to join our team! This role supports patient intake, scheduling, and coordination to ensure excellent care for patients and families.

Schedule Options:

• 16 or 24 hours per week


• Sunday: 8 hours


• Monday: 8 hours


• Optional Tuesday: 8 hours


• Hours: 8:00 AM – 4:30 PM

What You’ll Do:

• Manage patient intake and coordination


• Coordinate daily patient assignments and staffing


• Ensure continuity of care and fair staff assignments


• Prepare weekend and monthly schedules


• Support insurance verification and authorizations


• Assist with billing and documentation processes


• Build positive relationships with patients, families, and referral partners

What We’re Looking For:

• At least 1 year of experience (healthcare or hospice preferred)


• Experience with clinical records or coordination preferred


• Strong communication and organization skills


• Ability to work independently and collaboratively


• Compassionate, professional, and detail-oriented
