Job Description - Hospice LPN

JOB TITLE:                          Licensed Practical Nurse (LPN)


 


RESPONSIBLE TO:            Patient Care Manager



SUPERVISES:                      NA


 


POSITION SUMMARY:    The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a RN Case Manager and the Patient Care Manager.  Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. 


 


ESSENTIAL FUNCTIONS:




  1. Provides direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care.


 



  1. Implements current nursing practice following a comprehensive assessment and the plan of care.


 



  1. Provide accurate and timely documentation of patient services to reflect the plan of care.


 



  1. Provide patient and family/caregiver education and information consistent with the plan of care, considering learning abilities and readiness related to the patient’s health care needs and family’s coping styles and abilities.


 



  1. Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.


 



  1. Uses and prepares equipment and supplies effectively, adhering to aseptic technique.


 



  1. Offers optimal symptom management, emotional support, and energy saving techniques to patient and family. Offers support as needed for caregivers(s) so they may keep the patient at home or in the home setting if desired.  Evaluates patient’s response to treatments/medications. 


 



  1. Participates in personal, professional growth and development. Also participates in organization's quality assessment performance improvement teams and activities.


 



  1. Performs nursing skills according to the standards accepted within the State of Oregon.

QUALIFICATIONS:




  1. Graduate of an accredited practical nurse or vocational nursing program.


  2. Community health/hospice or medical/surgical experience is preferred.  New graduates with a passion for hospice should give us a call.


 



  1. Currently licensed as an LPN/LVN in the State.


 



  1. Complies with accepted professional standards and practice.


 



  1. Demonstrates good verbal and written communication, and organization skills.


 



  1. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.


 



  1. Understands philosophy of hospice concept. Also understands needs of the terminally ill.



  2. Proof of COVID Vaccination required per State of Oregon mandate. 


                                                                             


 


WORKING CONDITIONS/PERSONAL DEMANDS:





  1. Ability to function effectively in high stress and crisis situations. Ability to manage multiple priorities, and work as a team participant.


 




  1. Ability to assess and respond to the needs of patient and their families in any setting where the patient might be located.


 




  1. Demonstrates self-management skills related to personal loss issues in order to enable self to be an effective participant in the Hospice setting.


 




  1. Demonstrates computer literacy to accurately document patient visits as necessary.
