We are looking for a dedicated Weekend On-Call Hospice RN to join our team and help provide exceptional patient care.
Position Summary
The Weekend On-Call Hospice RN will be responsible for triaging patient needs, making home visits as necessary, and providing skilled nursing care during after-hours and weekend shifts. This role ensures continuity of care and timely response to urgent patient needs.
Key Responsibilities
- Respond to patient and family calls during assigned on-call hours.
- Assess patient condition and provide appropriate interventions.
- Conduct home visits as needed for symptom management, medication administration, and urgent care needs.
- Collaborate with the interdisciplinary team to coordinate ongoing patient care.
- Document all visits and communications in accordance with agency, state, and federal regulations.
- Provide emotional support and education to patients and families.
Qualifications
- Current Texas RN license in good standing.
- Minimum one year of hospice, home health, or related nursing experience (preferred).
- Strong clinical assessment and critical thinking skills.
- Compassionate and empathetic approach to patient care.
- Ability to work independently and manage time effectively.
- Reliable transportation and current auto insurance.
Schedule & Compensation
Weekend On-Call: Typically Friday 5:00 PM – Monday 8:00 AM.
