Hospice Social Worker, Part Time

Providence at Home is a growing division of Providence Life Services, which provides a continuum of healthcare, focused on senior living. Providence Hospice is seeking a Hospice Social Worker, Part Time in St. John, IN. We are looking for a compassionate Social Worker to join our established, experienced hospice team. We provide exceptional patient centered care to patients and families facing end of life. This position will work 24 hours per week.

Summary of essential job functions:

• Evaluate patient, family needs and support them through the end stages of life and to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
• Ordering Durable Medical Equipment and planning discharges from hospitals and/or nursing homes.
• Visiting patients and families on a weekly, bi-monthly, or monthly basis to provide ongoing counseling and support as patients are transitioning and dealing with end of life issues.

• Licensed Social Worker in the State of Indiana.
• Minimum of (1) year Home Health and Hospice experience preferred.
• Pass a physical, drug screen, 50-pound lift test, fingerprint for criminal background and complete the orientation program.
