Job Description - Hospice Social Worker | Part Time | Pierce County - 319BHR

We are seeking a compassionate and experienced Licensed Clinical Social Worker (LSWAIC or LICSW) on a Part Time basis to provide care for hospice patients in Pierce County, WA and surrounding areas. Services will be delivered in a variety of settings, including private homes, assisted living communities, and skilled nursing facilities.


We are looking for a Social Worker who is available for work on Mondays, Wednesdays and Fridays, from 8am -5pm. Flex for assessments and coverage needs.

Job Summary:


The Hospice Medical Social Worker (MSW) provides comprehensive psychosocial support to patients and their families facing end-of-life challenges. The MSW works as part of an interdisciplinary team to ensure that the emotional, social, and spiritual needs of patients are met while facilitating access to resources and support services and appropriate end-of-life care according to their needs and wishes.


 


Primary Responsibilities:



  • Conduct psychosocial assessments to identify patient and family needs.

  • Provide counseling for emotional, psychological, and grief-related issues.

  • Connect families with community resources and support services.

  • Collaborate with the interdisciplinary team to develop psychosocial care plans.

  • Educate on hospice services, end-of-life care, and coping strategies.

  • Offer crisis intervention and emotional support during emergencies.

  • Maintain timely, accurate documentation per regulatory standards.

  • Advocate for patient rights and preferences in care planning.

  • Participate in team meetings and contribute to care strategies.

  • Stay current with hospice best practices and continuing education.



Qualifications:



  • Education: Master’s degree in social work (MSW) from an accredited program.

  • Certifications: LSWAIC or LICSW

  • Experience: Experience in hospice or palliative care settings preferred, with a strong understanding of end-of-life care principles and practices.

  • Skills: Strong communication and interpersonal abilities, compassionate and empathetic approach to patient and family care, ability to work collaboratively within an interdisciplinary team, and excellent organizational and time management skills.

  • Certifications: Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) certification may be preferred depending on the setting.

  • Technical Proficiency: Proficiency with electronic health record (EHR) systems and other healthcare technologies.

Compensation: $65,000 - $88,000 per year, depending on experience.
All rates are negotiable within this range and will be discussed during the hiring process.
