Required Experience:

• A Registered Nurse or Licensed Practical Nurse, A Bachelor’s Degree in Social Work/Human Services/Related Field; or, A Bachelor’s Degree in any field not related with two years of human services experience; or an Associate’s Degree in Behavior Science/Social Science/Related Field with two years of human services experience.
• Experience as a case manager in SCL/MPW/ABI waivers preferred. 
• Mobile Worker: This is a mobile position, meaning that regular travel to different work locations is essential.  Will be exposed to weather conditions typical of the location and may be required to stand and/or sit for long periods of time.
• Reside in the same territory they are assigned to work in; exceptions may be considered, due to business need.
• Must be able to operate a computer and perform basic Windows functions for entering, updating, and maintaining individual’s Information System data. 
• Must have and maintain a valid state driver's license, maintain automobile insurance coverage, and have access to an automobile.
