Licensed Practical Nurse (Every Other Weekend)

Location: In Person – Fairfield County, CT
Pay Range: $40- $50 an hour

Position Overview:
Health Care Connectors is hiring a skilled Licensed Practical Nurse (LPN) to deliver direct care to patients of all ages in a home and outpatient setting. The LPN will support daily clinical functions, document care activities, and participate in patient education and discharge planning.

Key Responsibilities:

• Provide hands-on nursing care including medication administration and vital sign monitoring


• Assist with specialized procedures and clinical assessments


• Coordinate care plans with the healthcare team


• Document patient care using EHR systems (e.g., WellSky, HomeCare HomeBase)


• Educate patients and families on treatment plans and discharge instructions

Qualifications:

• Valid LPN license in Connecticut


• Experience with children and diverse clinical needs preferred


• Knowledge of sterile processing and infection control


• Effective communication and prioritization skills
