Requirements:

• ✔️ Minimum 1 year of LPN experience in an acute care setting (home care experience a plus)

• ✔️ Valid NY LPN license

• ✔️ BLS certification

• ✔️ Physical exam within the past year

• ✔️ PPD/QuantiFERON within the past year OR chest x-ray within 5 years

• ✔️ Driver’s license (preferred)

• ✔️ Authorization to work in the U.S.
