What You'll Do:

• 
  

  Deliver in-home care to patients recovering from illness or managing chronic conditions

  
  


• 
  

  Monitor vitals, conduct assessments, and report progress to the RN supervisor

  
  


• 
  

  Support the patient’s care plan and teach self-care techniques

  
  


• 
  

  Educate families on care routines, medications, and safety in the home

  
  


• 
  

  Collaborate with an interdisciplinary team to coordinate and optimize care

  
  

What You Need:

• 
  

  Active LPN license in Ohio

  
  


• 
  

  1 year of LPN experience (home health preferred)

  
  


• 
  

  CPR certification

  
  


• 
  

  Knowledge of state/federal home care regulations

  
  


• 
  

  Clinical confidence, excellent communication skills, and a compassionate heart
