We are a compassionate, client-centered home health care agency dedicated to delivering exceptional care in the comfort of our clients' homes. We’re currently seeking a Licensed Practical Nurse (LPN) to join our growing team for on-call and per diem home visits.
What You’ll Do:
- Provide skilled nursing care to clients in their homes under the direction of a Registered Nurse.
- Conduct timely and thorough assessments and deliver treatments based on physician orders and care plans.
- Educate clients and families on health conditions and self-care.
- Document all care activities in accordance with agency protocols and EMR requirements.
- Respond to client needs during assigned on-call shifts and per diem requests with professionalism and empathy.
- Collaborate with interdisciplinary teams to ensure continuity and quality of care.
Who You Are:
Licensed Practical Nurse (LPN) in the state of Connecticut
1+ year of nursing experience (home health preferred)
Strong communication, critical thinking, and documentation skills
Comfortable working independently in home settings
Reliable transportation, valid driver’s license, and auto insurance
Tech-savvy and familiar with EMR systems (training provided)
