Job Summary:

A qualified Licensed Practical Nurse (LPN or LVN) at FirstLight Healthcare Solutions, performs services as directed by the Registered Nurse.

Licensed Practical/Vocational Nurse (LPN/LVN) responsibilities include:

Provides skilled nursing services under the direction of a Registered Nurse.

Follows the plan of care and reports necessary revisions to the Registered Nurse.

Provides services in accordance with the plan of care.

Provides documentation of client assessments, plan of care, supervisory visits, caregiver and client education and training.

10. Informs client and/or client representative of changes in the condition and needs of the client.

Understands and adheres to established policies and procedures.

Teaches and counsels the client and family/significant others regarding their service needs and other related problems of the client at home when applicable.

Maintains accurate and complete records

Attends staff meetings, client care conferences, and in-services as scheduled.

Supervises and teaches other personnel, when applicable.

All other duties as assigned.

The ideal candidate will have:

Graduate of a state-approved school of nursing and currently licensed in the state in which practicing

Maintains CPR Certification

Minimum of one (1) year experience in Home Care, preferred.

Acceptance of the philosophy and goals of the Agency.

Ability to exercise initiative and independent judgment.

Compensation: $33.00 - $37.00 per hour
