Position Summary:

We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our team of dedicated healthcare professionals. The LPN will be responsible for providing clinical supervision and coordination of care for home health clients.

Responsibilities:

Monitor and evaluate clients' progress and adjust care plans as needed.

Educate clients and families on health management and self-care practices.

Collaborate with other members of the healthcare team to ensure coordinated and comprehensive care.

Maintain accurate and timely documentation of supervisory visits, interventions, and outcomes.

Promote a positive and supportive environment for clients and their families.

Qualifications:

Graduate from an accredited LPN program.

Current licensure as an LPN in the state of Georgia.

Minimum of one year of experience in home health care preferred.

Excellent clinical assessment and decision-making skills.

Strong communication and interpersonal skills.

Ability to work independently and as part of a team.

Valid driver's license1 and reliable transportation.
