Position Overview:

We are currently seeking Licensed Practical Nurses (LPNs) to join our team! In this role, you will be responsible for providing high-quality patient care by executing nursing processes, performing treatments, and supporting the physical and emotional needs of our residents. You will also work closely with our CNA team to ensure timely, accurate care, manage medication administration, and maintain thorough documentation.

  Pay Rate: $28.00 – $34.50/hr
  + Up to $2.00/hr Shift Differential!

  
  

LPN Responsibilities:

• 
  

  Patient Care & Service: Promote and restore patients’ health by completing the nursing process, performing treatment procedures, and providing physical, educational, and emotional support to patients, families, and friends. Supervise assigned team members.

  
  


• 
  

  Documentation: Ensure concise, pertinent, and complete resident care documentation using the computerized medical record system.

  
  


• 
  

  Support CNA Team: Direct CNAs in the timely completion of individualized personal care. Ensure CNA documentation is accurate and complete.

  
  


• 
  

  Medication Administration & Reporting: Administer medications and provide clear written and verbal reports in accordance with departmental protocols.

  
  

LPN Requirements:

• 
  

  Current and valid NY LPN License

  
  


• 
  

  Valid CPR Certification

  
  


• 
  

  Experience in a Nursing Home or Hospital is a plus
