What You’ll Do: As an LPN Home Health and Hospice Nurse, you’ll play a key role in our mission to provide outstanding care. Your duties will include:

Delivering skilled nursing care, such as wound care, catheter care, venipuncture, and medication management.

Educating patients and families on medication compliance and overall health care.

Using EMR charting to meet regulatory requirements.

Collaborating with a compassionate team, including home health and hospice staff, providers, and families.

What We’re Looking For:

A licensed LPN in the State of Utah.

CPR certification.

Strong organizational skills and attention to detail.

Ability to build rapport with patients and their families.

Problem-solving and critical-thinking skills.

Excellent communication skills, both verbal and written.

A valid Utah driver’s license, automobile insurance, and reliable transportation.

Preferred but Not Required:

Experience in home health and/or hospice care.

Compensation: $23.00 - $30.00 per hour
