Position Summary
We are seeking a PRN Master Social Worker (MSW) to join our hospice care team. The ideal candidate will provide emotional, psychosocial, and resource-based support to patients and families, helping them navigate end-of-life care with dignity and understanding.
Key Responsibilities
- Assess patients’ and families’ psychosocial, emotional, and support needs.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Provide counseling, crisis intervention, and grief/bereavement support.
- Assist families in accessing community resources and support services.
- Maintain accurate and timely documentation in accordance with state, federal, and agency guidelines.
- Participate in care conferences, team meetings, and patient/family education.
Qualifications
- Master’s degree in Social Work (MSW) from an accredited program.
- Current Texas licensure as LMSW.
- At least one year of hospice or palliative care experience preferred.
- Strong communication, counseling, and advocacy skills.
- Ability to work independently and collaboratively in a team setting.
Schedule & Compensation
PRN / As Needed – Flexible scheduling based on patient needs.
- Competitive PRN compensation.
