MCWP RN Case Manager

The Nurse Case Manager (NCM) plays a vital role in ensuring that Medi-Cal beneficiaries enrolled in the
Home and Community-Based Services (HCBS) Waiver Programs receive comprehensive and coordinated
care. The NCM will work collaboratively with the Medi-Cal Waiver Social Work Case Manager (SWCM),
beneficiaries, and their families to assess needs, develop individualized care plans, and facilitate access to
necessary services and resources within the community. This position is crucial in helping beneficiaries
maintain their health and independence in their homes with appropriate community-based interventions,
preventing unnecessary institutionalization. This person is adept at navigating complex systems of care.
The Nurse Case Manager treats clients in a way that aligns with the agency culture of acceptance, harm
reduction, and client autonomy.
