Job Description - MDS Coordinator Part time or Remote

Description

JOB SUMMARY

The primary responsibility of the MDS Coordinator is to oversee and facilitate resident assessments.

JOB DUTIES/RESPONSIBILITIES

The following job responsibilities will comply with the ADA and any/all applicable state and federal laws. Duties and responsibilities may change or new ones may be assigned at any time with or without notice.

  • Responsible for understanding and consistently following the principles of the Happy to Oblige (H to O) program.
  • Coordinate and ensure completion of MDS, RAPs, and ancillary assessments.
  • Coordinate with therapy to ensure appropriate care planning (ie. restorative rehabilitation and skilled therapy).
  • Complete Medicare assessment schedules.
  • Update census and condition sheets of residents.
  • Complete the RUGs communication forms and fax to central office as needed.
  • Audit charts for appropriate documentation supporting MDS and care plan.
  • Assist and train new personnel with regards to care plan and MDS.
  • Assist state surveyors during their visits.
  • Update kardex/faces sheet through care conference.
  • Coordinate overall development and implementation of the resident care plan.

Other duties may include:

  • Participate in Admissions Process (i.e. RUGs analysis)
  • Conduct care plan conference.
  • Coordinate family appointments.
  • Attend Medicare meetings.

WORK ENVIRONMENT

EQUIPMENT USED:

  • Rarely deals with dust, dirt, heat, cold, fumes, wetness, heights, and chemicals. Occasionally noise, humidity, and odors.
  • Equipment used: calculator, cart, pens/pencils, copy machine, fax machine, telephone, typewriter, and computer.
  • Employees may also be required to wear additional items as part of their uniform (ie: gait belts, back supports).

Requirements

QUALIFICATIONS

EDUCATION/EXPERIENCE:

  • Must be a licensed nurse in the State of Illinois.
  • Prior MDS/Care Plan experience preferred.
  • Continuing education as required.

KNOWLEDGE/SKILLS/ABILITIES

  • Familiarity with State and Federal regulations governing Long Term Care.
  • Effective communication, organizational, and critical thinking skills are required.
  • Enjoy working with older people, have a caring attitude and sincere interest in others; alert to attitudes and feelings of residents.
  • Computer literacy is a must.

PHYSICAL DEMANDS

  • Occasionally reaches above shoulders, crouches, kneels and balances.
  • Frequently bends/stoops, squats, pushes/pulls, lifts and carriers up to 11-25 pounds.
  • Continuously lifts and carries up to 10 pounds.
  • Walks and/or stands approximately 3-4 hours of each work day.
  • Clear speech, reading and hearing skills are required continuously to comprehend and communicate with residents and co-workers, to follow written plans of care and respond to emergency alarms.
  • Requires ability to read small print in typed and written materials.
  • Concentrates on moderate detail with some interruptions necessary to perform variety of tasks.
  • Comprehend oral and written instructions.
  • Memory capable of remembering task/assignment ranging from a few hours to long periods of time.
