Job Description - Meals on Wheels Assessor

Job Summary:  The MOW Assessor administers the state nutrition intake assessment, by meeting with senior clients or their caretakers in person or via telephone prior to enrollment on the Home Delivered Meal on Wheels Program.



Compensation: The hourly wage for this position is $23.00 (part-time; <19 hours/week).



FLSA: Hourly, Part-time, non exempt position



Schedule: Monday through Thursday between the hours of 7:30am - 5:30pm, depending on the individual program position needs.



Reports to: Meals on Wheels Program Manager

If selected for the position, applicants will be required to consent to background screening and/or drug and alcohol testing.



Duties and Responsibilities: 



  • Provide MAG program information to the community either in-person, electronically, or by phone

  • Meet with senior clients or their caretakers in person or via telephone to administer the state nutrition intake assessment prior to enrollment on the Home Delivered Meal on Wheels Program

  • Assist with the annual state nutrition assessment when enrolled in the Home Delivered Meal Program

  • Provide culturally relevant health education

  • Provide referral information to community resources

  • Serve as a bridge between the community and health care systems, social services, and the state health department

  • Improve access to health care services

  • Refer underserved community members to available services and resources

  • May participate in the MAG Caregiver Conference planning committee

  • Provide administrative support for the Department of Aging at the request of the department Director, which may include completing background screening processes for Paid Care attendants, ordering office supplies for the department, and other general office duties as assigned by the Director

  • Provide training to incoming part time staff and interns to perform administrative support and handle community phone inquiries



Supervisory Responsibilities: No supervisory responsibilities.



Required Knowledge, Skills, and Abilities:



  • Excellent communication skills, including the ability to respond to the public by phone, email, and in person

  • Punctual, responsible, detail-oriented, and able to complete work independently

  • Considerable knowledge of Microsoft Word, Excel and possess the willingness to learn new software applications

  • Ability to operate multi-line phone systems, standard office machines, & equipment

  • Highly organized and able to multitask well with interruptions

  • Must have, or be able to obtain, a valid Utah Driver’s License



Education and Experience: 



  • Graduation from high school and two years general business/office training, and

  • Two years work experience in an office setting, including data entry, knowledge of office procedures, bookkeeping and organizational skills

  • Or an equivalent combination of job-related experience and training

Physical Requirements: This position requires sitting at a desk or in meetings for extended periods of time. Regularly walk, stand, occasionally lift, carry, push, pull or otherwise move objects weighing up to 15 lbs., ability to project a voice for public speaking, and occasional exposure to high-stress environments. Occasionally drives a motor vehicle within Utah, Wasatch, and Summit counties.
