We are currently seeking a Part-Time Medical Social Worker (MSW) to join our patient-centered hospice team. If you have a heart for helping others navigate the complexities of end-of-life care and are looking to make a lasting impact in your community, we would love to hear from you.
Position Summary
As a Hospice Medical Social Worker, you will provide critical psychosocial support to patients and families, helping them cope with illness, grief, and end-of-life decision-making. You will serve as a vital bridge between the hospice team, the patient, and the community—ensuring that emotional, financial, and resource needs are met with compassion and expertise.
Key Responsibilities
- Accept and respond to referrals from the interdisciplinary team for patients requiring social work services
- Conduct assessments to evaluate patients’ psychological, emotional, social, and economic needs
- Plan and provide short-term, goal-oriented counseling and support to patients and their loved ones
- Connect patients and families to appropriate community resources and support services
- Maintain accurate and timely documentation in accordance with the Plan of Care and hospice policies
- Participate in interdisciplinary case conferences and provide psychosocial input to care plans
- Make scheduled home visits and participate in on-call rotation as needed
- Uphold patient rights and dignity, promote comfort, and support end-of-life transitions with empathy and professionalism
What You Bring
Education:
Master’s degree in Social Work (MSW) from a school accredited by the Council on Social Work Education (CSWE)
Experience:
- Minimum 1 year of social work experience, preferably in a healthcare setting
Licensure & Certification:
- Current license to practice as a Social Worker in the state of New Mexico
- CPR certification
- Valid driver's license with current auto liability insurance
Skills & Qualities:
- Strong interpersonal and counseling skills
- Excellent communication, documentation, and time management skills
- Knowledge of Medicare/Medicaid and community resources
- Ability to work independently in home settings
- Professionalism, empathy, and emotional resilience
