Nurse Practitioner

Join our compassionate team as a Hospice Nurse Practitioner (PRN)!

We’re seeking a Nurse Practitioner (PRN) in Saint Louis/Creve Coeur, MO to provide specialized end-of-life care in patients’ homes, including comprehensive assessments, plan of care management, and face-to-face visits for admission, recertification, or evaluations. This role is critical in guiding patients and families through their hospice journey, offering both clinical expertise and emotional support.

Key Responsibilities:

• Deliver palliative and hospice care in collaboration with an interdisciplinary team.
• Conduct assessments and develop individualized care plans.
• Provide hospice certification and recertification evaluations.
• Coordinate with physicians, staff, and families to ensure seamless care.
• Document all patient interactions and maintain compliance with regulatory standards.

Qualifications:

• Nurse Practitioner degree from an accredited program.
• Active NP and DEA license in MO, with at least 1 year of hospice experience.
• Excellent communication and teamwork skills.
• Valid driver’s license and reliable transportation.

Job Details:

• PRN position, paid per face-to-face visit ( $150), plus mileage.
• Service area includes Jefferson, Franklin counties, and nearby areas.
