Nurse Practitioner – Home-Based Care

Varco Home Health and Hospice is seeking a compassionate Nurse Practitioner to provide primary care to patients in their homes. Under physician supervision, you will perform health assessments, manage acute and chronic conditions, educate patients and families, and coordinate care to ensure positive outcomes.

Responsibilities:

• Conduct health histories, physical exams, and diagnostic assessments

• Develop and implement individualized treatment plans

• Prescribe medications and make referrals per guidelines

• Document patient encounters in EHR within 72 hours

• Educate patients/families on disease management, preventative care, and wellness

• Communicate effectively with physicians, care teams, and patients

Qualifications:

• Graduate of an accredited Nursing program

• Current RN and Nurse Practitioner license

• NP experience (home health or hospice preferred)

• Valid driver’s license and reliable transportation

• Strong clinical, communication, and organizational skills

Work Environment:

• Patient homes with variable conditions; possible exposure to infectious diseases

• Flexible schedule with occasional PRN calls

• Ability to lift up to 50 lbs and assist with patient mobility
