Essential Job Functions

As an Occupational Therapist, you will support clients in maximizing their independence and improving their quality of life through personalized therapy interventions in the home setting.

🔹 Conduct patient evaluations and develop customized care plans focused on functional goals.
🔹 Deliver skilled therapy to help clients improve ADLs, IADLs, strength, coordination, and safety.
🔹 Educate patients and caregivers on safe practices, equipment use, and home modifications.
🔹 Collaborate with physicians, nurses, and other care team members.
🔹 Maintain accurate documentation and assist with certification/recertification paperwork.
🔹 Support quality improvement efforts and participate in optional on-call rotations.

Requirements

✔ Active Occupational Therapist (OTR/L) License in Minnesota
✔ Two-Step TB Test (provided if needed)
✔ DHS Criminal Background Clearance (we provide screening)
✔ Valid Driver’s License & Auto Insurance
