We are currently looking for a part-time Occupational Therapist to support our Older Adult Home Modification Program. This is a part-time opportunity, up to 14 hours/week.

This position is funded through March 31, 2028.

SUMMARY

The Occupational Therapist will serve as a key member of the Older Adult Home Modification Program (OAHMP) team, responsible for conducting initial and follow-up home assessments for older adults, recommending and documenting home modification interventions, and providing education on installed equipment. This role involves close collaboration with clients, organization staff, and contractors to ensure that all modifications align with grant requirements, agency regulations, and best practices. The ideal candidate is a detail-oriented, motivated professional committed to supporting older adults in maintaining independence and safety in their homes. The position is funded through January 11, 2028.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The duties and responsibilities of the Occupational Therapist will include but are not limited to the following:

• Conduct initial and follow-up in-home assessments to evaluate safety, accessibility, and functional needs of older adults.
• Determine eligibility for home modifications under the OAHMP grant and provide recommendations to ADAPT.
• Make recommendations for home modifications and accessibility improvements to support aging in place.
• Document assessments using Home for Life Design tool and submit timely reports to funders and program leadership.
• Ensure compliance with federal, state, local, and HUD regulations related to accessibility and home modifications.
• Participate in interdisciplinary team meetings and required program reviews.
• Provide referrals to additional community services as needed.
• Lead or support community outreach efforts to promote program availability and recruit eligible participants, as needed.
• Develop and implement quality control procedures to ensure high standards of services and client satisfaction.
• Perform other related duties as assigned to support the success of the program.

QUALIFICATIONS

• Bachelor of Science in Occupational Therapy. Masters preferred.
• Current NY State license in Occupational Therapy
• Five years of prior Occupational Therapist experience preferred.
• Certification or specialized training in home modifications or aging-in-place (e.g., CAPS – Certified Aging-in-Place Specialist).
• Familiarity with grant-funded or nonprofit service delivery models.
• Excellent interpersonal skills are required to interact effectively with employees, co-workers, families, other departments, outside contacts, etc. 
• Ability to work independently and handle multiple tasks simultaneously.
• Excellent oral and written communication skills. 
• Strong initiative, an eagerness to learn, and the ability to work both as part of a team and independently. 
• Ability to maintain confidentiality. 
• Ability to prioritize assigned tasks and travel to, and between programs, administrative, and project locations.
• Excellent computer skills, including Microsoft Office.
• Passion for ADAPT's mission and demonstrates a commitment to the non-profit disability sector.

COMPENSATION: $83.72/hour, 14 hours/week, grant-funded through March 2028
