Position Summary

The Outreach Coordinator plays a vital role in building and maintaining relationships with referral sources, healthcare partners, and community organizations. This individual will promote Interim Healthcare’s services, support business development efforts, and help connect clients and families with the care they need. The ideal candidate is a natural communicator with a passion for community engagement and a strong understanding of healthcare services.

Key Responsibilities

  Develop and maintain strong relationships with referral sources including hospitals, physician offices, skilled nursing facilities, assisted living centers, and community agencies.

  Represent Interim Healthcare at community events, health fairs, and professional networking opportunities to promote brand awareness.
  
  

  
  


• 
  

  Collaborate with internal teams to ensure timely and appropriate follow-up with potential clients and referral partners.

  Support marketing initiatives, including outreach campaigns, educational presentations, and digital engagement efforts.

  Track outreach activity, maintain accurate records, and report on referral trends and outcomes.

  Identify new outreach opportunities and partnerships that align with Interim Healthcare’s mission and growth goals.

  Maintain a strong understanding of Interim Healthcare’s services and local market needs to effectively communicate offerings.

  Establish a strong, visible presence within the community by participating in local coalitions, councils, or professional associations.

  Serve as a brand ambassador for Interim Healthcare, consistently communicating the organization’s mission, values, and commitment to quality care.

  Assist in organizing or sponsoring community events that align with Interim Healthcare’s outreach goals and brand image.

  Maintain an updated CRM or outreach log documenting all contacts, activities, and outcomes.

  Participate in regular business development meetings to review goals, share insights, and plan next steps.

  Develop and implement targeted outreach strategies to increase referrals in key service lines (e.g. home health, hospice, personal care).

Qualifications

• 
  

  Experience in healthcare outreach, marketing, community relations, or case management strongly preferred.
  
  

  
  


• 
  

  Excellent communication, presentation, and relationship-building skills.
  
  

  
  


• 
  

  Self-motivated, organized, and able to manage multiple priorities.
  
  

  
  


• 
  

  Proficiency in Microsoft Office and CRM systems.

  Valid driver’s license, reliable transportation, and willingness to travel locally.
