All Homecaring Florida, a trusted home health agency based in the Twin Cities and now expanding in Florida, is seeking a compassionate and dedicated RN Case Manager to serve clients in Wesley Chapel, Florida, and surrounding areas. This position starts as part-time with the opportunity to transition to full-time as caseload grows.

Your Role:
As an RN Case Manager, you will plan, organize, and deliver personalized home care services. Responsibilities include:
Conducting comprehensive physical assessments and reassessments.
Developing, implementing, and updating individualized care plans.
Administering prescribed treatments and medications.
Educating and empowering patients and their families.
Supervising home health aides and Personal Care Assistants (PCAs).
Mentoring new RNs to foster professional growth.
Participating in a rotating on-call schedule.
Collaborating with physicians, staff, and caregivers.
Ensuring compliance with Florida regulations and accurate documentation.
Requirements:
Current RN License in Florida: Active and in good standing with the Florida Board of Nursing.
Florida RN Licensing Requirements:
Graduate of an accredited nursing program (Associate’s or Bachelor’s degree in Nursing).
Successful completion of the NCLEX-RN exam.
Completion of a Commission on Graduates of Foreign Nursing Schools (CGFNS) evaluation, if educated outside the U.S.
Current CPR certification.
Compliance with Florida’s continuing education requirements (24 hours every 2 years, including 2 hours on prevention of medical errors and 2 hours on domestic violence).
Valid Driver’s License and Proof of Auto Insurance.
Ability to Pass a Level 2 Background Check: As required by the Florida Agency for Health Care Administration (AHCA), facilitated by All Homecaring Florida.
2-Step TB Skin Test or Blood Draw: We can provide testing if needed.
