Job Description - Part Time RN - Medication Management - Edgewater, Chicago, IL



Monday - Friday 9am - 4pm


Partial List of Responsibilities:



  • Provide guidance and training to In Home Caregivers 

  • Conduct new client assessments

  • Create new client care plans

  • Conduct regularly scheduled client re-assessments and update care plans

  • Communicate care plan updates and recommendations to clients and family members

  • Work closely with Independent Community operations leaders to maintain client referrals

  • Fill in for In Home Caregivers for client ADL activities if and when necessary

  • Companionship and social interaction, engaging in conversation, games, and activities
