Licensed Practical Nurse (LPN)
Schedule: Weekends, 8 hr shifts
Duties and Responsibilities:
- Provide skilled nursing care to patients in their homes
- Conduct comprehensive assessments of patients' health status
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patients' progress and adjust care plans accordingly
- Educate patients and their families on managing their conditions and promoting wellness
- Collaborate with other healthcare professionals to coordinate patient care
- Document patient care activities accurately and timely
Experience:
- Preferred minimum of 1 year of experience as an LPN in a home health setting
- Experience with tube feeding and managing enteral nutrition
- Knowledge of sterile processing techniques and infection control protocols
- Experience working with children in a healthcare setting preferred
