You’re a compassionate, board-certified APRN licensed in the state of New Hampshire with a passion for whole-person, community-based care. What You'll Do:

• Provide outpatient pediatric care with a focus on prevention, wellness, and the management of acute and chronic conditions.
• Perform assessments of physical, psychosocial, and developmental status for patients and families.
• Deliver education, counseling, and emotional support to patients and caregivers.
• Document care thoroughly in our electronic health record system (EHR).
• Collaborate with behavioral health clinicians, community partners, and support staff to meet the full spectrum of your patients’ needs.
• Participate in our shared on-call rotation.
