Job Description - Physical Therapist

Position Overview:

We are currently seeking a dynamic and compassionate PRN Physical Therapist to join our team. The PRN Physical Therapist will provide rehabilitative services to individuals in their homes, focusing on enhancing mobility, reducing pain, and improving overall physical function. This is an excellent opportunity for a skilled Physical Therapist who values the autonomy of home health care and is passionate about making a positive impact on the lives of our clients.

Responsibilities:

Conduct comprehensive assessments of patients' physical abilities and limitations in a home setting.

Develop and implement individualized treatment plans based on assessment findings and in collaboration with the healthcare team.

Provide skilled physical therapy interventions, including therapeutic exercises, manual therapy, and modalities, to achieve treatment goals.

Educate patients and their families on home exercise programs and techniques to maximize independence.

Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.

Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure coordinated and holistic care.

Maintain accurate and timely documentation in accordance with regulatory and company standards.

Participate in team meetings, case conferences, and continuing education opportunities.

Qualifications:

Current license to practice as a Physical Therapist in the state of Virginia.

Minimum of [X years] of clinical experience as a Physical Therapist.

Home health care experience is preferred but not required.

Strong clinical and interpersonal skills.

Excellent communication and documentation abilities.

Demonstrated ability to work independently and as part of a collaborative team.

Current CPR certification.

Compensation: $100.00 - $120.00 per hour
