Position Overview
Virginia Home Care Services is seeking a PRN Physical Therapist to join our dedicated team of professionals. This flexible, per-diem role offers the opportunity to provide meaningful care and make a direct impact on patients' lives through in-home physical therapy services.

Key Responsibilities

Perform comprehensive assessments to evaluate patients' physical condition and functional abilities.

Develop and implement individualized treatment plans based on clinical findings and patient goals.

Utilize appropriate therapeutic exercises, techniques, and modalities to enhance mobility, strength, and overall function.

Educate patients and caregivers on therapeutic activities, proper techniques, and strategies to support independence and prevent injury.

Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.

Collaborate effectively with interdisciplinary team members—including physicians, nurses, and occupational therapists—to ensure coordinated and high-quality care.

Maintain accurate, timely documentation in accordance with regulatory standards and company policies.

Qualifications

Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program.

Minimum of 2 years of professional experience as a licensed Physical Therapist.

Active Physical Therapy license in the state of Virginia.

Current CPR certification.

Strong clinical skills with a solid foundation in evidence-based physical therapy practices.

Excellent communication, interpersonal, and organizational skills.

Demonstrated ability to work independently and manage a varied caseload.

Compassionate, patient-centered approach to care.

Prior experience in home health or community-based settings preferred.

Experience with Home Care Home Base (HCHB) is a plus.
