The Physical Therapist (PT) evaluates, diagnoses, and treats individuals with functional movement, mobility, strength, balance, and physical performance impairments while delivering resident-centered physical therapy services that support rehabilitation and functional independence. The PT works collaboratively with patients, families, caregivers, and the interdisciplinary care team, and provides clinical supervision and oversight of Physical Therapy Assistants (PTAs) as applicable. Services are delivered in accordance with Pennsylvania licensure requirements, professional standards, and organizational policies, with a strong focus on dignity, safety, and individualized care.

Physical Therapists (PTs)Responsibilities:
Clinical Evaluation and Treatment
Screens, examines, and evaluates patients through medical history review, systems review, and application and interpretation of appropriate tests and measures to establish a diagnosis and plan of care.
Develops measurable treatment goals and intervention plans in collaboration with patients, families, caregivers, and the interdisciplinary care team.
Implements physical therapy treatment plans and adapts programs based on patient needs, progress, tolerance, and clinical judgment.
Provides therapeutic services including, but not limited to:
Application of electrotherapeutic, physical agent, and mechanical modalities
Performs routine re-examinations and progress assessments to modify or advance treatment plans as clinically indicated.
Supervision and Delegation
Provides clinical supervision, direction, and delegation to Physical Therapy Assistants (PTAs) in accordance with state licensure requirements, organizational policy, and professional standards.
Ensures delegated tasks are appropriate to the PTA’s scope of practice and patient plan of care.
Maintains accountability for patient outcomes and services delivered under delegated care.
Specialized and Program Services
Designs, fabricates, fits, applies, and trains patients in the use of adaptive equipment and assistive devices, including orthotics and prosthetics.
Selects therapeutic activities appropriate to the individual’s physical and cognitive abilities, condition, and interests.
Instructs patients and caregivers in skills and techniques necessary for safe participation in mobility, functional, and therapeutic activities.
Evaluates patient progress, attitudes, behaviors related to rehabilitative potential and functional outcomes.
Interdisciplinary Collaboration and Operations
Collaborates with members of the rehabilitation and interdisciplinary care teams to select and implement appropriate therapeutic interventions.
Attends and actively participates in patient care conferences, staffing meetings, and other related interdisciplinary meetings.
Makes referrals to other disciplines or external agencies to address comprehensive patient needs.
Ensures effective and efficient delivery of rehabilitation services, including adherence to established plans of care, patient schedules, and departmental expectations.
Completes all clinical documentation accurately and timely in compliance with payer, regulatory, and organizational requirements.
Performs other related duties as assigned.
Physical Therapists (PTs)Minimum Qualifications:
Current licensure or eligibility for licensure as a Physical Therapist in the Commonwealth of Pennsylvania.
One of the following educational qualifications:
Master’s degree in Physical Therapy; or
Master’s degree in a Physical Therapy–related field with a Bachelor’s degree in Physical Therapy; or
Bachelor’s degree in Physical Therapy with a minimum of five years of progressively responsible Physical Therapy experience.
Knowledge of physical therapy principles and evidence-based practices delivered in a resident-centered care environment.
Ability to evaluate, treat, and document physical therapy services while respecting patient rights, dignity, and individual preferences.
Ability to collaborate effectively within an interdisciplinary care team.
Must be able to communicate the English language effectively both orally and in writing, and comprehend the same.
