 Physical Therapist Career Opportunity: We are seeking a Physical Therapist to join our team on a part-time or PRN basis. The PT will provide in-home speech therapy services for residents in senior living communities as well as individual homes throughout Anderson and surrounding counties. Key Responsibilities:

• Provide Physical Therapy to patients in line with individual client guidelines and CPHC standards.
• Screen and evaluate patients to determine the need for intervention and develop comprehensive treatment plans with clear goals.
• Communicate evaluation findings and treatment plans to patients, families, and other team members, coordinating schedules and care.
• Implement and adjust treatment plans based on patient response, adapting methods to meet individual needs and physician orders.
• Develop home or community programs to support patients' progress in their own environments.
• Maintain accurate documentation of assessments, treatments, progress, and discharge plans.

Requirements

• Must hold active Indiana Physical Therapist license
• CPR certification (ACLS preferred)
• One year of experience in home health setting required
