About the Role:

The Post-Acute Care Physician will provide in-person medical care to patients in skilled nursing and rehabilitation facilities. Responsibilities include managing acute and chronic conditions, facilitating care transitions, collaborating with facility staff, and supporting value-based care initiatives.

Core Responsibilities:

• 
  

  Perform in-person assessments and manage treatment plans in skilled nursing facilities

  
  


• 
  

  Collaborate with interdisciplinary teams to ensure continuity and quality of care

  
  


• 
  

  Update documentation and EMR in a timely and accurate manner

  
  


• 
  

  Participate in Trumen’s value-based and transitional care programs

  
  


• 
  

  Build and maintain strong relationships with facility staff and administration

  
  


• 
  

  Be available on-site during scheduled days 

  
  

Qualifications:

• 
  

  MD or DO with a current, unrestricted Texas medical license

  
  


• 
  

  Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine

  
  


• 
  

  Minimum of 1 year of experience in post-acute, geriatric, or SNF settings preferred

  
  


• 
  

  Strong clinical judgment, communication, and organizational skills

  
  


• 
  

  Reliable transportation within the Tyler area
