Primary Care Nurse Practitioner - Part Time - Columbia

Key Responsibilities:

As a Primary Care Provider, you will provide primary care to patients in long-term care facilities. This includes:

·         Developing and implementing personalized treatment plans based on patient needs.

·         Offering medication management, including prescribing and monitoring medications.

·         Collaborating with a multidisciplinary team of healthcare professionals to ensure patient care.

·         Maintaining accurate medical documentation in line with regulatory standards.

·         Providing education and support to patients and their families regarding mental health conditions and treatments.

Qualifications:

• Current Nurse Practitioner license in specified location.

• At least two years of experience is required

• Experience providing care to geriatric populations in long-term care settings is preferred but not required.

• Excellent communication skills, especially when working with facility staff and residents' families.

• Ability to manage time effectively, including flexibility in scheduling and being responsive to patient needs.
