Are you a compassionate and skilled APRN looking to make a meaningful impact in the lives of patients and families during their most critical time? We are seeking an experienced Advanced Practice Registered Nurse to join our dynamic Hospice team! In this vital role, you will provide expert nursing care and evaluate patients within the framework of our mission and philosophy of Hospice.

Working alongside the Interdisciplinary Hospice Team and Hospice Medical Director, you'll gather, document, and share clinical findings to assess hospice eligibility and patient status. You will perform essential face-to-face encounters for hospice patients at key intervals, ensuring they receive the best care possible throughout their journey. This position is working on a PRN basis. 

Key Responsibilities:

  • Evaluate and manage hospice patients, providing nursing care and ensuring continuity of care.
  • Collaborate with the interdisciplinary team and Hospice Medical Director to determine continued eligibility and patient status.
  • Conduct face-to-face encounters for recertification and ongoing assessment, ensuring timely reviews every 60 days.

  • Master’s Degree in Nursing from an accredited school of nursing
  • AANP or ANCC board certification 
  • Valid driver’s license 
  • Current automobile insurance 
  • Reliable vehicle
