Job Description - PRN RN Case Manager Home Health (High Point, Archdale, Thomasville)

Caring Hands Home Health, Inc. is seeking a dedicated and compassionate PRN RN Case Manager to support our clients in High Point, Archdale, and Thomasville, NC.

Key Responsibilities:

Conduct in-home assessments and develop care plans

Educate patients and caregivers on treatment plans and health management

Perform supervisory visits every 90 days or 60 days for CAP cases

Document patient interactions in a mobile EMR system

Travel within High Point, Archdale, Thomasville, and surrounding areas

Collaborate with field staff and office team to ensure high-quality care

Qualifications:

Active RN license in NC or a compact/multi-state license

Graduate of an accredited nursing program

Minimum of 2 years of nursing experience (home health preferred)

Strong clinical judgment and communication skills

Reliable transportation and mobile device for documentation

Ability to work independently and travel for client visits

Location:
This position serves clients in High Point, Archdale, Thomasville, NC and surrounding areas.

Compensation:
Competitive hourly rate based on experience. Weekly pay. This is a PRN (as needed) position.
