Job Description - Quality Improvement Program Assistant

Quality Improvement Program (QIP) Assistant



Title: Quality Improvement Program (QIP) Assistant
Location: Fortuna Family Medicine
Reports To: QIP Supervisor
Employment Type: Part-Time, Hourly - 20 hours weekly With the potential to go to Full time in 3-4 months 
Schedule: Negotiable/Flexible



Position Summary


The Quality Improvement Program (QIP) Assistant supports the clinic’s Quality Improvement initiatives by conducting patient and staff outreach and maintaining accurate tracking of outreach attempts. This role works collaboratively with the QIP team to communicate progress in scheduling and outreach. 


This position has minimal compliance responsibilities and is primarily focused on quality improvement outreach, tracking, communication, and follow-through.



Key Responsibilities


Outreach & Follow-Up



  • Conduct patient outreach related to QIP initiatives (e.g., preventive care gaps, follow-up needs).

  • Follow up with QIP Department Supervisor on outstanding QIP items.

  • Relay QIP-related questions and updates to QIP Coordinator and Supervisor

  • Scheduling appointments, tracking records and results



Collaboration & Communication



  • Work hand-in-hand with the QIP team to facilitate the achievement of department standards and goals.



Process Improvement Support



  • Support clinic-wide quality improvement initiatives as assigned.



Skills and Qualifications



  • Strong organizational and tracking skills with high attention to detail

  • Excellent communication and follow-up skills

  • Ability to manage multiple priorities and deadlines

  • Comfortable working with data, reports, and spreadsheets

  • Collaborative team player



Education and Experience



  • High school diploma or equivalent required

  • Prior healthcare or medical clinic experience preferred

  • Experience with quality improvement, care coordination, outreach, or performance tracking preferred

  • Familiarity with QIP measures, RHC/FQHC environments, or clinical quality metrics is a plus
