Registered Nurse

Position Summary: 

Under the supervision of the Senior Clinical Services Manager, the Registered Nurse (RN) is responsible for carrying out functions within the RN scope of practice. This is a part-time position; 24 hours/week

Duties and Responsibilities:

• Provide nursing services including health assessments, first aid, flu shots, health education and coaching, referrals to medical and community resources, and patient advocacy.
• Maintain confidential health records. 
• Assess patients’ current health status, resource utilization, past and present treatment plan and services, prognosis, short and long-term goals, treatment and provider options.
• Coordinate services between Physicians, specialists, medical providers, and non-medical staff as necessary to meet the complete medical socio-economic needs of clients. 
• Provide patient education. 
• May perform procedures using Roots Standardized Procedures.
• Actively participate in integrated team care management rounds.
• Monitor referrals made to community-based organizations, medical care and other services to support the patients’ overall care management plan. 
• Provide oversight of a population of complex, high-risk patients, including monitoring identification of those eligible, referral rates, care plan completion, and patient outcomes. 
• Enter and maintain assessments, authorizations, and pertinent clinical information into Electronic Health Record systems.
• Attend departmental and organizational meetings as required.

 Competencies: 

• Valid, current California license as a registered nurse, current AHA CPR certification and either of the following: A Bachelor’s Degree in Nursing and a California Public Health Nurse certificate and a minimum of two (2) years’ experience in nursing; 
• Understanding of, sensitivity to, and respect for the diverse academic, socioeconomic, cultural, disability and ethnic backgrounds of community members.
• Willingness to work a flexible schedule.
• Ability to establish and maintain effective working relationships with individuals and groups, both professional and non-professional, co-workers, management personnel, the public and others.
• Solid organizational skills including attention to detail and multi-tasking.
• Clear and effective external and internal, verbal and written, communication skills.
• Ability to work with people from diverse backgrounds.
