Registered Nurse – Medicaid Waiver Program (HCS) – DFW / North Texas Travel

Pay: $40–$44/hr (1099 Contractor)

Overview

A Texas Heart For Families, Inc., a Texas-licensed Home and Community Services (HCS) provider, is seeking a dependable and detail-oriented Registered Nurse (RN) to perform annual Medicaid Waiver program assessments for clients served by Certified Nursing Assistants (CNAs) in the Dallas–Fort Worth / North Texas area.

This PRN, 1099 contract position offers competitive pay, flexible scheduling, and the opportunity to work independently while helping clients remain in their homes and communities rather than in institutional care.

Responsibilities

• Conduct in-person initial assessments and annual CNA skills check-offs for Medicaid waiver clients in accordance with HCS program requirements.

• Develop individualized, person-centered service plans and ensure the client’s health and welfare are protected.

• Complete documentation on paper or iPad; submit securely to the Allen, TX office for compliance and record-keeping.

• Schedule and coordinate visits independently to meet program timelines.

• Travel within the DFW / North Texas region (company vehicle available from Allen, TX office or mileage reimbursement for personal vehicle use).

• Provide occasional PRN skilled nursing visits as needed.

Requirements

• Current Texas RN license (compact license accepted).

• Strong clinical assessment, documentation, and organizational skills.

• Willingness to travel within the DFW / North Texas area.

• Ability to work independently and manage your own schedule.

• Experience with Medicaid waiver programs or HCS preferred (training available for motivated RNs).
