Registered Nurse (RN) Case Manager - Home Health

We are seeking a dedicated and compassionate Home Health Registered Nurse Case Manager to join our team. The ideal candidate will be responsible for providing high-quality nursing care to patients in their homes, coordinating their care plans, and ensuring they receive the necessary services to promote their health and well-being.

Key Responsibilities:

• Conduct comprehensive assessments of patients' health status and needs.
• Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and healthcare teams.
• Monitor patients' progress and adjust care plans as necessary.
• Provide direct nursing care, including medication administration, wound care, and health education.
• Coordinate with interdisciplinary teams to ensure continuity of care and effective communication.
• Educate patients and families on disease management, medication adherence, and health promotion.
• Maintain accurate and timely documentation of patient care and progress.
• Ensure compliance with healthcare regulations and standards.
• RN nurse to help with start of cares and visits

Skills and Qualifications:

• Current Registered Nurse (RN) license in the state of practice.
• Bachelor’s degree in Nursing (BSN) preferred.
• Previous experience in home health, case management, or a related field.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Proficient in electronic health records (EHR) and other healthcare technologies.
• Compassionate and patient-centered approach to care.
• 1 year experience in Home Health preferred

• PRN weekday 
• PRN weekends
