What We Offer
Integrated Healthcare Coordination (IHCC) – Work closely with physicians, nurse practitioners, therapists, and social workers to develop and implement individualized care plans for home visit clients.
Comprehensive Adult Day Services – Provide care in a structured, engaging environment that promotes well-being, independence, and socialization.
Expanding Home Visit Services – Deliver personalized nursing care, medication management, and chronic disease support in a home setting.
Responsibilities
Integrated Healthcare Coordination (IHCC) – Home Visit Services
- Provide individualized nursing care in clients’ homes, focusing on comfort, safety, and independence.
- Administer medications and chronic disease monitoring while ensuring compliance with care plans.
- Conduct comprehensive health assessments and adjust care plans as needed.
- Coordinate with physicians, therapists, and case managers to ensure seamless healthcare services.
- Educate clients and families on disease prevention, self-care techniques, and medication adherence.
- Maintain accurate and timely documentation in compliance with healthcare regulations.
Adult Day Center – Community-Based Nursing Care
- Monitor and support daily health and wellness for clients in a structured day program.
- Administer medications and perform routine health checks, documenting changes in condition.
- Assist clients with mobility, personal care, and activities of daily living (ADLs) while promoting independence.
- Work collaboratively with the Adult Day Center team to implement social, recreational, and therapeutic activities.
- Respond effectively to medical emergencies and ensure client safety.
- Foster a welcoming, inclusive environment that promotes dignity and engagement.
Compensation & Shift Differentials
Base Pay: $25.00 to $29.00 per hour
Schedule: 2 days at home and 2 days in office or in the field.
Qualifications
Active LPN license in the state of Indiana
CPR certification (or willingness to obtain)
- Experience in home care, adult day services, long-term care, or integrated healthcare coordination preferred
- Strong ability to collaborate with healthcare professionals and ensure continuity of care
- Excellent communication and documentation skills
- Ability to work independently in home settings and collaboratively in a team-based adult day center
- Passion for high-quality, compassionate care and client advocacy
