Position Overview

As a PRN Home Health Nurse, you will provide skilled nursing care to patients in the comfort of their homes. This role is ideal for a nurse who values autonomy, personalized care, and the opportunity to build strong patient relationships.

Key Responsibilities

• Deliver professional nursing care in patients’ homes
• Perform wound care and manage treatment plans
• Conduct patient admissions and comprehensive assessments
• Develop and implement individualized care plans
• Administer medications and prescribed treatments
• Monitor patient progress and communicate changes to the healthcare team
• Educate patients and families on health conditions and care procedures
• Maintain accurate, timely documentation of all care provided

Position Details

• Status: PRN (as needed)
• Schedule: Flexible, based on your availability
• Location: Home-based care within our service area

Licensure: Current valid Iowa Registered Nursing License

Education: Associate degree with relevant experience

Experience: Two years of hospice, long term care, assisted living or hospital related work experience preferred

Driving: Proof of license and current insurance coverage

- Excellent communication and interpersonal skills

- Ability to work independently and as part of a team

- Compassionate and patient-centered approach to care
