JOB DESCRIPTION

We are looking for a team member who is energetic, very well organized, able to multi-task, has a warm telephone manner and excellent customer service skills.

A strong work ethic and the ability to interact well with a wide variety of people.

It is a perfect opportunity for a self-starter who enjoys a blend of detailed office work and hands on caregiving in the patient’s home.

This is a key position to ensure the agency is compliant with NJ Board of Nursing regulations and our accreditation status.

QUALIFICATIONS

Graduate of an accredited school of professional nursing. BSN, PHN preferred.

Current license as a Registered Nurse in the state(s) of practice.

Minimum of one (1) year experience in an acute care setting or equivalent experience.

Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair.

Current CPR certification.

COVID 19 vaccination required.

Strong communication and interpersonal skills.

RESPONSIBILITIES

Performing comprehensive initial assessments of client status, including physical, psychosocial, and environmental parameters.

Developing an individualized plan of care for each client with client input.

Providing interventions with a focus on achieving realistic client outcomes.

Conducting client reassessments and follow up visits.

Maintaining compliant client records showing systematic assessments, planning interventions, and evaluations.

Provide training, delegation, supervision and support to our staff of caregivers.

Address any issues with respect to clients and or caregivers that may arise.

Flexible work from home options available.

Compensation: $0.16 - $0.17 per hour
