Description

SUMMARY:

This person functions as a direct staff person, providing care, supervision and training for individuals within Residential (i.e. Group Home or Supported Living) Services at Achievements, Inc. 

Exemption Status: Non-exempt

Immediate Supervisor: Group Home Manager, Supported Living Manager and/or SS Manager

Direct Reports: None

Essential Duties and Responsibilities:

  1. Teach and demonstrate services to individuals with developmental disabilities, including functional community-based training opportunities. Assist in assessing, designing, implementing, documenting, and evaluating Individual Teaching Protocols, including behavioral programs. May be required to present plans and findings at PSP meetings. Follow and ensure compliance with in-house Quality Assurance plan or timely and complete preparation of PSP meeting information. Collect, record, and analyze program data for individuals. Record and analyze periodic case notes.
  2. Participate in routine operations of the facility to which assigned. This may include minor facility maintenance and cleaning, providing transportation for individuals, grocery shopping, menu planning, preparation of meals, assisting individuals with personal care activities (i.e. bathing, grooming, toileting, etc), coordinating and ensuring proper medical/dental appointments as assigned, activities in the community, handling financial transactions.
  3. Record/chart and submit data as required. 
  4. Participate in providing safety training to individuals. Observe all safety rules and follow safe job procedures. Maintains clean and safe working environment in facility/facilities to which assigned, participate in regular facility maintenance schedules, fire drills and follows and ensures compliance to corporate safety policies and procedures.
  5. Assist in establishing and maintaining working relationships with families, guardian, and advocates of the individuals, with neighborhood residents, co-workers and with representatives of public service agencies.
  6. Conducts self in a professional, respectful, trustworthy manner toward individuals in services, co-workers, supervisors, etc. Positively represents individuals in services and Achievements.
  7. Assists in maintaining compliance of assigned facility and services to standard prescribed by Achievements, Inc., the Department of Public Health and Human Services, the Developmental Disabilities Program, Accreditation Agencies and any and all licensing agencies
  8. Other related duties as assigned.

Requirements

Required Minimum Qualifications:

  1. This position requires education and experience equivalent to a high school diploma and completion of occupationally specific training. Experience providing educational, vocational, residential, or community services for persons with disabilities beneficial but not necessary.
  2.  Ability to acquire knowledge of the habilitation process, training program management and practices, policies and procedures for DPHHS-DDP habilitation training, principles of community integration and normalization and techniques of non-aversive training acquired through completion of identified sections within the College of Direct Support.
  3.  Ability to acquire skill and certification in First Aid, Cardio-Pulmonary Resuscitation (CPR), managing non-aggressive people (MANDT System), pass and maintain certification to assist with medications. 
  4.  Must be capable of giving, receiving, interpreting, and applying verbal and/or written instructions and directions.
  5.  Good work habits, including but not limited to honesty, punctuality, maintenance of confidentiality, professionalism and the ability to organize and manage time and information.
  6.  Valid Montana Driver’s License, maintenance of a good driving record, and insurability for operation of company vehicles.
  7.  Ability to lift objects up to 50 pounds throughout shift. Ability to perform tasks requiring repetitive motion.

Job Demands

  1. During a normal 8-10 hour shift, employee will be standing up and moving at least 80% of the time. 
  2. Bending, lifting, carrying, pushing, pulling, etc. are common motions to be expected.
  3. Walking and working surfaces are normally even.
  4. Exposure to unpleasant sights, sound, odors, verbal and physical aggression may be encountered.
  5. Supervise individuals and assist in daily activities and personal care. Chart documents and activities as required
  6. Assists with the supervision of individuals administering their medications. Follow established protocols for assisting with administration and/or checking of medications. Chart as required. 
  7. Cope with stressful circumstances and cordially, professionally and respectfully interact with individual in services, co-workers and supervisors as a member of a team.
  8. Maintain confidentiality of individuals in services as well as organizational confidentiality. 
  9. Physical effort is required, but not limited to physically assisting individuals in daily activities, range of motion, bathing, toileting, transferring, utilize MANDT physical skills if necessary, carry supplies, perform household cleaning tasks, and other tasks as necessary. 
  10. Plan and assist in a variety of leisure and recreational activities
  11. Both indoor and outdoor activity is required
  12. Implement individual training programs, record data, perform assessments, write case notes and GER’s. 
  13. Accepts directions from supervisors and follow through with directives
  14. Communication information effectively, both verbally and in writing
  15. Operation of a computer
  16. Perform duties requiring repetitive motion
  17. Lift from floor and to/from shoulder level 
  18. Maybe required to alter scheduled hours or days off as necessary
  19. Operate corporation vehicles to provide transportation to individuals as needed. Accurately document required information.
  20. Frequently lift up to 50 lbs
