RN Case Manager (Home Health) Delray Beach, FL

AMG Home Healthcare Services is currently seeking a part-time RN Home Health Case Manager with prior work experience with Medicare patients. This is a part-time position at our Home Healthcare office located in Delray Beach, FL. Opportunity to transition to Full-Time role may be discussed if desired 

Case Manager is responsible for managing, supervising, assisting, and evaluating the activities of the clinical staff and assigned patient census for the Home Health agency. Please see position details below:

DUTIES AND RESPONSIBILITIES

Duties of the RN Case Manager include, but are not limited to:

• Provides supervision of field nurses, HHA/CNAs & professional staff to assure the quality of services and progress towards patient goals


• Reviews the clinical content, diagnosis, medical history, medications, and other procedures of the record of patients


• Assists with teaching and counseling to enhance the delivery of services and Professional development of staff.


• Identifies and evaluates nursing needs of patient referrals and the appropriateness of home services by reviewing & determining if referral meets admission criteria


• Assists with the orientation of new staff & participates in competency assessments of new staff.


• Completes OASIS-E Start of Care documentation within 24 hours of admitting new patients. May need to make routine home visits to assigned patients as needed.


• Participates in patient update meetings with Corp team, patient/company audits, & interdisciplinary team conferences.


• Develops Plan of Care for assigned SN patients and develops Aide Care Plan(s) for patients with Aide services. Amends POC as needed

POSITION REQUIREMENTS:

• Must have 1+ year(s) of prior work experience as a Home Health Case Manager with Medicare patients


• Must have reliable transportation and the ability to conduct home visits on a regular basis 


• Knowledgeable and experienced in using WellSky/Kinnser EMR
