RN Home Health Care

Elevate Healthcare Services is seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health Care team in Virginia Beach, VA. In this role, you will provide high-quality nursing care to patients in their homes, helping them maintain their health and independence in a comfortable and familiar environment. This position offers a competitive compensation of $44.00 - $46.00 per hour and the opportunity to make a meaningful impact on the lives of our patients and their families.

Responsibilities

• Deliver comprehensive nursing care to patients in their homes, including assessment, treatment, and education.


• Develop and implement personalized care plans based on patient needs and physician instructions.


• Monitor patient progress and report any changes in condition to the healthcare team.


• Administer medications, injections, and other treatments as prescribed.


• Provide education and support to patients and their families regarding disease management, health promotion, and prevention.


• Maintain accurate and timely documentation of patient care and communication.


• Collaborate with interdisciplinary team members to optimize patient outcomes.

Requirements

• Current Registered Nurse (RN) license in the state of Virginia.


• Minimum of 1 year nursing experience, preferably in home health care or a related setting.


• Strong clinical assessment and critical thinking skills.


• Ability to work independently and manage time effectively in a home setting.


• Excellent communication and interpersonal skills.


• Valid driver’s license and reliable transportation.


• CPR certification required.
