JOB SUMMARY
Abernathy Home Care is hiring a compassionate and reliable RN or LPN to provide supervisory visits for HHAs in client homes. This is not an in-office position — nurses travel throughout our service area. Mileage and travel time are paid. This role is perfect for nurses who value flexibility, independence, and meaningful client care.

ESSENTIAL JOB FUNCTIONS

• 
  

  Complete supervisory visits for Home Health Aides (HHAs)

  
  


• 
  

  Conduct client assessments and update care plans as needed

  
  


• 
  

  Provide occasional hands-on nursing care (med setup, vitals, simple treatments)

  
  


• 
  

  Document visits in AxisCare EMR

  
  


• 
  

  Communicate changes in client condition to the care team

  
  


• 
  

  Maintain professional relationships with clients, families, and staff

  
  


• 
  

  Participate in weekend visits at least every other weekend

MINIMUM QUALIFICATIONS

• 
  

  Active Massachusetts RN or LPN license

  
  


• 
  

  Valid driver’s license and reliable transportation

  Strong assessment and communication skills

  
  


• 
  

  Comfortable working independently in client homes

  
  


• CPR Certified
