Key Responsibilities:

• Conduct initial in-home patient evaluations and Start of Care visits
  
• Assess health status and safety concerns; recommend services as needed
  
• Document thoroughly and accurately using OASIS standards
  
• Provide patient and caregiver education about ongoing care plans
  

What You Need to Bring:

• Active RN license in Minnesota
  
• Home Health Nursing and OASIS admission experience
  
• Exceptional independence, professionalism, and clinical documentation skills
  
• Access to reliable transportation and valid insurance
