Job Title: Per Diem COTA – Weekend Community -Based Rehab

Company: Home Health Care, Inc.

Schedule: Saturdays and/or Sundays (as available)

Pay: $42 - $46 per visit.

Mileage reimbursement between clients!

Duties

• Perform prescribed therapy activities and mobility training in the patient’s home.
  
• Monitor and report changes to the Occupational Therapist.
  
• Reinforce therapeutic goals and ensure safety in ADL routines.
  
• Help patients and caregivers understand adaptive equipment and care strategies.
  

Skills

• MN COTA license.
  
• Ability to work independently.
  
• Commitment to quality care delivery.
  
• Detail -oriented documentation.
