InterCare is searching for a part-time School Based Therapist to join our team at the Washington Street Elementary School in Otsego! Minimum Requirements
- Master's Degree in Social Work, Psychology, Counseling or Human services filed.
- Must have LLMSW, LMSW or LPC licensure in Michigan; LLMSW or LMSW licensure preferred
- Experience with Motivational Interviewing and CBT.
- CAADC Certification preferred
Primary Accountability
As a licensed clinician, the School-Based Therapist will provide CBT (Cognitive Behavioral Therapy) and other EBPs (Evidence Based Practices) at School districts which have a contract with InterCare, for children and adolescents of ages 5 through 21 years which are aimed at achieving the best possible physical, intellectual and emotional health status. In addition, the School-Based Therapist may provide targeted Mental Health and SUD disorder treatment services to children, adolescents and adults at the clinical sites, as needed.
Description of Primary Duties & Responsibilities
- Provide behavioral health services to youth in InterCare’s school-based sites when the school is open, per E3 program requirements.
- Maintain a minimum caseload of 50 students annually.
- The service delivery plan must be reflective of the needs of the school and may include the following components/services;
- Treatment groups using evidence-based curricula and interventions.
- School staff training and professional development relevant to mental health.
- Building level promotion such as school climate initiatives, bullying prevention, suicide prevention programs etc.
- Classroom education related to mental health topics.
- Case management and partnerships with other private/public social service agencies.
- A comprehensive risk assessment will be completed for every student which will include a depression screen.
- Obtain appropriate consent forms to provide behavioral health services, from parents/ legal guardians/ students etc. and obtain Release of Information (ROI) form for care coordination/case management purposes.
- Complete any registration forms, as needed.
- Track any required data, complete monthly reports etc.
- Provide consultation and assistance to medical providers on the management of behavioral health concerns in primary care and help implement the team-determined treatment plan (when assigned to a clinical site).
- Conduct comprehensive screenings to evaluate the needs of adults, adolescents, and children for behavioral health intervention, case management, or ongoing mental health or addiction treatment. Provide referrals for more intensive services where appropriate.
- Provide behavioral interventions to engage and motivate Patients to actively manage their care. Conduct follow-up as appropriate based on assessment and treatment. Provide crisis management for Patients as needed; make linkages for interventions as appropriate.
- Participate in ongoing training in behavioral health issues and provide consultation and training to primary care clinic staff in mental health and behavioral medicine topics including; communication skills, brief interventions, counseling skills, medication use and misuse, substance abuse, wellness, rehabilitation, recovery and self-sufficiency.
- Establish and maintain a current listing of community linkages to meet the needs of the target population. Develop and maintain collaborative working relationships with community providers. Initiate linkages, disseminate information and make referrals to the appropriate services as needed.
- Conduct patient satisfaction surveys with patients receiving behavioral health services under the program. Also, participate in program model development and evaluation, and collect and compile data as required by program model leads.
- Participate in regular multidisciplinary case conferences with medical providers as needed. Complete population health projects as assigned and panel-manage patients in collaboration with other behavioral health staff.
- Meet adopted provider performance expectations.
- Meets established peer review outcomes.
- Meets daily productivity expectations (5 billable encounters per day).
- Meets documentation standards in the EHR.
- Meets patient and team satisfaction expectations.
- Any other duties as assigned by the Director of Behavioral Health.
Requirements

- Ability to be part of the school environment when possible, attending events, parent teacher conference, staff meetings, allowing school staff to understand your role and the services of the program.
- Must coordinate personal time off requests with the needs of the school scheduled summer/holiday breaks, to the extent feasible, for optimal utilization of therapy availability.

Professional and Technical Knowledge
- Maintains a working knowledge of PCMH, and IHI care coordination and integration standards.
- Clinical experience with provision of mental health services in a community-based setting.
- Knowledge of, and experience working in, a primary care facility as a mental/behavioral health specialist preferred.
- Understands and consistently supports the uniform implementation of InterCare’s relevant clinical guidelines and organizational policies and procedures in assigned areas.
- Focuses self and team members on functional outcomes for patients that improve quality of life and reduce unnecessary or inappropriate utilization of the health care system.
Technical Skills
- Proficient in Microsoft Office Suite
- Ability to use clerical and numerical skills in preparing final drafts and documents from raw data.
- Ability to create letters, forms, documents, and presentations.
- Ability to run reports, analyze and interpret data, assist in implementation and maintenance of HR systems.
Communication Skills
- Possesses a professional level of written and verbal communication skills.
- Ability to communicate complex concept in a clear effective manner.
- Possesses excellent cross cultural communication skills and the ability to communicate to staff members at all levels.
- Proficient English and Spanish speaker preferred.
Physical Demands
Job duties are performed in the typical office environment of the organization, which requires ordinary ambulatory skills sufficient to visit other locations. The environmental factors and/or physical requirements of this position include the following:
- Ability to stand, walk, stoop, kneel, crouch, frequently sit, and lift up to 25 lbs.
- Requires good hand-eye coordination, reach with arms & hands, and finger dexterity, including ability to grasp, and manipulate (using keyboard, office equipment).
- Visual acuity to use keyboard, operate office equipment, and read printed material and regularly required to talk and hear.
